The PASSPORT/C.H.O.R.E. Program

Passport
The East Toledo Family Center offers transportation services to and from medically related appointments.  The PASSPORT program is a state funded program operated by the Area Office on Aging of Northwestern Ohio.  This program assists qualifying seniors with such things as:
  • Personal Hygiene
  • Home Delivered Meals
  • Housekeeping and Homemaking
  • Medical Transportation
  • Emergency Response Systems
  • Adult Day Care Services
  • Medical Equipment and Supplies
  • Counseling / Social Work
  • Planning for Long Term Care
Participants must be at least 60 years of age, residing in a non-institutional setting and meet financial eligibility guidelines.

 

C.H.O.R.E. / Housekeeping Services
The East Toledo Family Center offers services through this program to East Toledo residents who are at least 60 years of age.   This program is also funded, in part, through grants from the Area Office on Aging of Northwestern Ohio.
Services provided include the following:
  • Lawn Care
  • Landscaping
  • Garage and Basement Clean-up
  • Snow Removal
  • Light Housekeeping Services

Minor home repair services include:

  • Interior Painting
  • Wall and Ceiling Repair
  • Carpentry
  • Minor Electrical Repairs
  • Plumbing

Fees for Chore, housekeeping and minor home repairs are $9.00 per hour for members and $15.00 per hour for non-members.  Membership to the East Toledo Family Center or  The East Toledo Senior activities Center will qualify for membership rates.  Membership rates are $15.00 for individuals and $25.00 for family memberships.
*CHORE and Housekeeping services are limited to two times per month.

Major Home Repair Program

Lucas County residents over 60 years of age and qualifying financially are eligible for the major home repair program which is funded by the Area Office on Aging of Northwestern Ohio.  Participants must be homeowners and qualify according to income guidelines.  Applications are available at the East Toledo Family Center.  After Completing an application the application must be submitted to the Area Office on Aging when completed. Applicants will be notified of eligibility when the review process is completed.  Grant limits are 1,403.00 per household.  The program assists seniors in maintaining their residence in a safe habitable condition.  All requests for repairs are considered for eligible clients.   Services are provided through the East Toledo Family Center.

Food Assistance

The Family Center offers a monthly surplus commodity distribution for area residents.  Distribution is at the East Toledo Senior Activities Center on the second Tuesday of each month.  Delivery is available for seniors who are restricted to their homes an unable to pick up their commodities. This distribution is available to all people in need.

These are available at the Family Center.  Eligibility is according to need.  Food supplies are limited to once monthly.  Referral to Toledo Seagate Foodbank is also available at the Family Center. Referral to Toledo Seagate Foodbank is also available at the Family Center.
