Pathways Programs
Pathways is a program designed for expectant mothers to help reduce the number of low birth weight deliveries in Lucas County. Expectant mothers are assigned to a Care Coordinator who’s responsibilities include mentoring and monitoring the pregnant client’s prenatal medical care including establishing a medical home and setting/keeping medical appointments to improve birth outcomes. Our Care Coordinator will attend medical visits and make apointment reminders by phone and/or mail as needed as well as provide valuable Parent Education prior to the baby’s arrival. In addition, the Care Coordinator will help link families to resources in the community. The Care Coordinator will continue to work with the mom after the child is born until her post-partum visit in which at this time the family will have completed participation in the program and becomes eligible for a program completion incentive.
